Anti–mGluR5 antibodies
Test details
This laboratory assay detects IgG autoantibodies specific to mGluR5, a receptor expressed on neurons, especially within the limbic system and hippocampus. Metabotropic glutamate receptors (mGluRs) are G-protein–coupled receptors activated by glutamate, the main excitatory neurotransmitter in the CNS. There are eight mGluR subtypes (mGluR1–8) classified into three groups by their molecular, pharmacologic and signalling properties. mGluR5 and mGluR1 belong to Group I, are predominantly postsynaptic, and are involved in learning, memory, epilepsy and pain. Autoantibodies to these receptors are linked to several autoimmune CNS disorders.
Anti-mGluR5 antibodies are found in a distinct form of autoimmune limbic encephalitis known as Ophelia syndrome, often featuring behavioural and cognitive disturbances and associated with Hodgkin lymphoma in about half of cases. Typical manifestations include personality and behavioural changes, memory and attention deficits, and sometimes seizures or status epilepticus. In more severe cases, altered consciousness and hypoventilation may occur. Early diagnosis and immunosuppressive therapy can lead to partial or complete remission in most cases.
