SARDs are a heterogeneous group of disorders characterized by dysregulation of the immune system, that starts to turn against the body’s own tissues. Major SARDs include systemic lupus erythematosus, rheumatoid arthritis, systemic sclerosis and Sjögren’s syndrome. These diseases can affect multiple organs and share common symptoms such as fatigue, fever, joint pain and skin rash. Diagnosis relies on a combination of clinical assessment and specific autoantibody tests. Early diagnosis is crucial to initiate appropriate therapy and slow down disease progression.

 

The Sm (Smith) antigen is a complex of nuclear polypeptides associated with small nuclear ribonucleoproteins (snRNPs) involved in pre-mRNA processing. Anti-Sm antibodies are autoantibodies directed against this component and are a highly specific marker for systemic lupus erythematosus (SLE).

 

Although present in about 15–30% of SLE patients, anti-Sm antibodies are highly specific and rarely found in other autoimmune diseases. For this reason, they are included in the SLE classification criteria of both the ACR (American College of Rheumatology) and SLICC (Systemic Lupus International Collaborating Clinics).
